The owner or operator of any well shall notify the supervisor or the district deputy, in writing, in such form as the supervisor or the district deputy may direct, of the sale, assignment, transfer, conveyance, or exchange by the owner or operator of such well, and the land, owned or leased, upon which the well is located, within 30 days after such sale assignment, transfer, conveyance, or exchange. The notice shall contain the following: (a) The name and address of the person to whom such well was sold, assigned, transferred, conveyed, or exchanged. (b) The name and location of the well. (c) The date of the sale, assignment, transfer, conveyance or exchange. (d) The date when possession was relinquished by the owner or operator. (e) A description of the land upon which the well is situated.
