The notice of intention to abandon shall contain the following information: (a) The total depth of the well to be abandoned. (b) The complete casing record of the well, including plugs. (c) Such other pertinent data as the supervisor may require on printed forms supplied by the division or on other forms acceptable to the supervisor.
