(a) All notices of cancellation shall be in writing, mailed to the named insured at the address shown in the policy, or to the insured’s last known address, and shall state, with respect to policies in effect after the time limits specified in Section 676, all of the following: (1) Which of the grounds set forth in Section 676 is relied upon. (2) In accordance with the requirements of subdivisions (a) and (e) of Section 791.10, the specific information supporting the cancellation, the specific items of personal and privileged information that support those reasons, if applicable, and corresponding summary of rights. (3) On or after July 1, 2020, a notification that if the policyholder believes the policy has been wrongfully canceled, the policyholder may have the matter reviewed by the department. The notification shall include the department’s internet website, www.insurance.ca.gov, the department’s telephone number, 1-800-927-HELP (4357), and the mailing address of the department’s Consumer Services Division, 300 South Spring Street, Los Angeles, CA 90013. (b) For purposes of this section, a lienholder’s copy of those notices shall be deemed mailed if, with the lienholder’s consent, it is delivered by electronic transmittal, facsimile, or personal delivery.
