The policy shall contain a provision that: (a) The policy, the application of the employer and the individual applications, if any, of the employees constitute the entire contract of insurance. (b) All statements made by the employer or by the individual employees are, in the absence of fraud, representations and not warranties. (c) Such statements will not be used in defense to a claim under the policy, unless contained in a written application.
