For births that are being registered under this chapter there shall be required documentary evidence and affidavits pursuant to one of the following: (a) Two pieces of documentary evidence, at least one of which shall support the parentage. (b) One piece of documentary evidence and one affidavit executed by the physician or other principal attendant. (c) One piece of documentary evidence and two affidavits executed by either the mother, father, or other persons having knowledge of the facts of birth.
