The State Department of Health Care Services and State Department of State Hospitals shall promulgate a standard written consent form, setting forth clearly and in detail the matters listed in Section 5326.2, and any further information with respect to each item as deemed generally appropriate to all patients. The treating physician shall utilize the standard written consent form and in writing supplement it with those details which pertain to the particular patient being treated.
