The consent described in subdivision (c) of Section 1011 shall be obtained by filing an application with the commissioner in a form to be prescribed by him or her accompanied by that additional information concerning the insurer, its condition and affairs, as the commissioner requires. A fee of six thousand three hundred fifty dollars ($6,350) shall be paid to the commissioner for the filing of the application.
