An application for registration pursuant to Section 9884 shall designate that the applicant is registering as an auto body repair shop if the applicant intends to perform auto body repair. In addition, an application for registration to operate an auto body repair shop shall include a written statement signed under penalty of perjury that the applicant has been issued licenses or permits, if required by law, including, but not limited to, all of the following: (1) A city or county business license. (2) A State Board of Equalization identification or resale permit number. (3) An Environmental Protection Agency hazardous waste permit number. (4) An Air Quality Management District spray booth permit number.
